Hannah is in prison (having been put there by Dexter, after she poisoned his sister, Debra).
Hannah
phones Arlene (her former roommate at the reformatory) from prison, and
tells her that she can't live the rest of her life in prison.
She asks Arlene to help her end it, and at first we imagine that Hannah (an expert on poisons) plans to commit suicide.
After Hannah reminds Arlene that she saved her from the abusive counselor, Arlene asks Hannah to tell her what to do.
Later, Arlene slips her a white pill in court.
But it turns out that she uses the pill to bring on a temporary seizure, then uses the medical emergency as an opportunity to escape.

A. This scene was shot near the southwest corner of N. Beachwood Drive & Lexington Avenue, in Hollywood.
Arlene is parked next to the green wrought-iron fence (and stop sign) at 1201 Beachwood, facing south, as you can see in this matching Google StreetView.
Dexter producers love
to cluster-shoot, so it's no coincidence that they shot this scene just
a few houses south of the apartment where Dexter visits Estrada's wife.
